SECTION IV
This section is concerned with describing the individual. Your observations should be as complete as possible and should be based on your own observations and on information obtained from others who know the individual well.
The statements on the page below indicate various traits of personality that apply to most people. On the left side of the page is a list of statements. On the right side are six categories. For each statement, check the category that applies to the individual. If the statement does not apply to the individual, check "Does Not Apply." If you have not observed the individual sufficiently to make a judgment, check "Not Observed." If the statement applies to the individual, check the category that indicates the degree to which the statement applies. If the statement applies to an average degree, check "Applies to an Average Degree." If the statement applies to a degree above average, check "Applies to an Above Average Degree." If the statement applies to an unusual degree, check "Applies to an Unusual Degree."
