SPECIFIC DUTIES
List up to six of the most important specific duties performed during the rating period. Insert rating letter which best describes the employee's performance in each duty. Rating letters must be inserted in Section B, "Performance Evaluation," opposite the appropriate duty number. Specific duties must be listed in order of their relative importance in terms of the employee's overall performance.

The rater must comment briefly about the employee's effectiveness in his current position such as general performance, strengths, weaknesses, and any other factors which influenced his overall performance during the rating period. Based on your knowledge of employee's overall performance during the rating period, place the letter in the rating box corresponding to the statement which most accurately reflects his level of performance.
