This form provides the means whereby your official personnel record will be kept current. Even though all information requested may have been previously submitted, it is essential that you complete this form in detail. If additional space is required, use a separate sheet of paper and attach it to this form. If you have been transferred since your last report, submit this form to your new commanding officer. If you have not been transferred, submit this form to your commanding officer. If you have been separated from the service, submit this form to the Commandant of the Marine Corps (Code AO). If you have been separated
