SECTION A: PERFORMANCE OF SPECIFIC DUTIES
DIRECTIONS:
1. Rate each of the specific duties listed below as to the extent to which SECRET duties are PERFORMED in the present position.
2. Consider the individual’s performance of each duty in terms of the requirements of the position and the extent to which he meets these requirements.
3. Rate each duty separately, using the following scale:
1 - UNACCEPTABLE: Performance is clearly below the minimum acceptable level.
2 - MARGINAL: Performance is barely acceptable.
3 - ACCEPTABLE: Performance meets the minimum requirements of the position.
4 - GOOD: Performance is above average.
5 - EXCELLENT: Performance is clearly superior.
6 - OUTSTANDING: Performance is exceptional.

DIRECTIVES: Stress strengths and weaknesses, particularly those which affect development on present job.
The ability to obtain a high degree of cooperation from important industrial and educational sources in his area; ability to plan and direct the operations of his office with efficiency and exceptionally high standards; complete integrity. This integrity, coupled with a somewhat legalistic approach to many problems, tends to make him somewhat inflexible in his approach to the sometimes unusual requirements of intelligence, particularly when they concern clandestine matters.
SECTION B: SUITABILITY FOR CURRENT JOB IN ORGANIZATION
DIRECTIONS: Take into account everything you know about the individual—production, conduct on the job, etc.—and rate him as to his suitability for his present job.
